Our Programs


The Prevention Program, operated in collaboration with partner agency LEAVEN, provides financial assistance, connection to community resources, and follow-up services to individuals and families who are at high-risk of losing their community housing. Most people who are helped by Prevention maintain their community housing and do not return within one year for additional financial assistance.


The Street Outreach Program connects individuals who are acutely homeless and living with mental illness and/or addiction challenges to community resources and housing. Outreach also assists those who are eligible for social security income with the application process. The Street Outreach Program is operated in collaboration with the Fox Valley Warming Shelter.

Homeless Connections houses six chronically homeless individuals identified through the Street Outreach Program in permanent, affordable housing units at 319 N Appleton Street.


The Shelter Program provides safe, temporary shelter to up to 78 men, women, and children who are experiencing homelessness. Residents are also provided with personal care items and meals to meet their basic needs.


The Case Management Program provides personal guidance to individuals and families in the Shelter Program. Case managers assess each individual, identifying barriers to self-sufficiency. Individuals then work with their case managers to develop a plan with goals to address these barriers and connect to community resources like workforce development, health care, and housing providers.

Homeless Connections also brings select services in-house to best meet the needs of our residents. These include: financial coaching with FISC, legal services from Legal Action, a nurse practitioner and therapist from Partnership Community Health Center, an Economic Support Specialist from Outagamie Health and Human Services, and occupational therapy students from Fox Valley Technical College.

Educational Services

Homeless Connections also partners with Fox Valley Technical College to offer an Adult Basic Education program for adults who are homeless or formerly homeless at the City Center.
